Returning and Pre-Registered Students

RETURNING AND PRE-REGISTERED STUDENTS

Step 1 - Read the following documents.  As required by law this information is made available annually to parents or guardians of a child enrolled in a school maintained by Scotts Valley Unified School District:

  •   Superintendent's Letter to Parents and Students
  •   Annual Notice to Parents      
  •   California Immunization Requirements for TK/K-12
  •   Elementary/Middle School Promotion/Retention Policy
  •   High School Promotion/Retention Policy
  •   Middle School Progress Reports
  •   High School Progress Reports
  •   Elective Absence Fund Letter
 

Step 2 - Print the Acknowledgement Signature Form below and return to your child's school site along with the following required documents (one form per student):

NOTE:  If you are having difficulty printing the forms, try using the "Ctrl P" command to open the print dialog box (hold down the "Ctrl" key, and the letter "P")

  •  Signature Acknowledgement Page
  •  Student Residency Questionnaire
  •  Photograph Permission Form
  • California Healthy Kids Survey (CHKS) Consent Form (required for 5th grade)
  • California Healthy Kids Survey (CHKS) Opt-Out Form (for 7th, 9th & 11th grades - only if you do not  want your child to participate in the survey)
  • Note:  The CHKS forms are not required for grades 1, 2, 3, 4, 6, 8, 10, or 12
 

Step 3 - Update your student's information: 

  1. Access the Illmuninate Home Connection Portal at Illuminate Home Connect
  2. Log in with your username and password (For help try SVUSD IT Help Desk
  3. Once in the portal, select the forms that need updating and make the necessary changes.  Possible changes include address, phone numbers, email addresses, health information and emergency contacts (a mimimum of two).  Contact your school office if you do not see any forms.
  4. Click "Submit" to enter your changes.
 

Step 4 - Free and Reduced Price Meals - This information is in your registration packets, but you can easily check the eligibility guidelines and print the application here as well.  If you are unsure if you qualify even after reviewing the guidelines, you should go ahead and apply anyway.
