Returning and Pre-Registered Students


Step 1 - Read the following documents.  As required by law this information is made available annually to parents or guardians of a child enrolled in a school maintained by Scotts Valley Unified School District:


Step 2 - Download and completed the fillable forms listed below, then print and return to your child's school site (one form per student):

NOTE:  If you are having difficulty printing the forms, try using the "Ctrl P" command to open the print dialog box (hold down the "Ctrl" key, and the letter "P")


Step 3 - Update Student and Parent Information in Illuminate (please ensure that parent(s)/guardian(s) cell numbers and email addresses are accurate so that important communication is not missed): 

  1. Access the Illuminate Home Connection Portal at Illuminate Home Connection
  2. Log in with your username and password (For help try SVUSD IT Help Desk
  3. Once in the portal, select Forms on the left and find form titled 2020-2021 - Update Student Information. If there is no form available contact your school office. Possible changes include address, phone numbers, email addresses, health information and emergency contacts (a minimum of two).
  4. Click the green button that says, "I'm finished. Submit Now" even if you have not made any changes.

Step 4 - Free and Reduced Price Meals - This information is in your registration packets, but you can easily check the eligibility guidelines and print the application here as well.  If you are unsure if you qualify even after reviewing the guidelines, you should go ahead and apply anyway.
